The Anatomic Graduated Component (AGC) total knee system from Biomet was first implanted in 1983. This is a minimally conforming “flat on flat” design attributed to Ritter. The system includes Universal and sided cobalt-chrome femoral components. The tibial component is available as one piece or modular variants, and has a stem which has an I-shaped cross-section. There are cemented and uncemented (the AGC 2000) variants.

 
More stabilised variants include the Cam-and-Groove Posterior Stabilised variants or standard P/S.
 
In the 2010 UK National Joint Registry report, the AGC accounted for 11% of primary total condylar knee replacements, behind DePuy’s PFC Sigma and Zimmer’s NexGen and ahead of Smith and Nephew’s Genesis 2. The three and five year revision rates were 2.1% (1.9% to 2.3%) and 3.0%(2.7% to 3.3%) respectively, which in crude terms placed it behind the PFC, Nexgen, Triathlon and others, and just below the mean.
 
In Ritter’s series of 7760 knees, 36 had follow up over 20 years, with an estimated survival of 97.8%. Success was attributed to the durability of the non-modular compression moulded polyethylene metal-backed tibial component and the relatively unconstrained articular geometry.
