The Corail stem has been in clinical use since the 1980s. It is a fully hydroxy-apatite coated (150 micron thickness) non-porous forged titanium alloy stem. The design includes a proximal trapexoidal cross section, intended to resist axial/torsional stresses and promote initial stability, and a tapered distal stem which is quadrangular and provides a stiffness gradient, rather than cortical contact. The shoulder profile is reduced, and there are proximal horizontal and distal vertical grooves on the surface. The surgical technique is bone preserving – broaches impact rather than removing bone.

It is available in collared and collarless (see image) versions. The collared version may be preferred when there is a crack in the calcar or concern about stability. There are standard and high offset options and a 12/14 taper.

This stem has an excellent track record, with a number of papers quoting long-term survival, including 97% survivorship in 5,456 cases at 15 years (1). The stem is most often paired with the Pinnacle or Duraloc uncemented cups. Good results have been reported even for use in patients with fractured neck of femur and osteoporosis.

The device was the UK market leader in uncemented stems in the 7th National Joint Registry report, with 47% of the market and had a 5-year revision rate of 3.8%(3.3% to 4.3%) (2). The device has an ODEP 10A rating.
