Flexible nailing of forearm fractures:
For patents aged 7 -15 years old
Avoid in adults as healing is slower which slows rehabilitation
nail is always placed in a retrograde approach in the radius to avoid the risk of damage of PIN
2 approaches for the distal radial insertion point:
Distal lateral insertion risk of damage to the superficial branch of the radial nerve
dorsal approach over Lister’s tubercle 2 cm proximal to the distal physis
in the ulna the nail can be inserted via an antegrade (3cm distal to the tip of the olecranon) or retrograde approach.
The nail diameter 2/3 of the medullary isthmus – usually 2 mm in 7-11 years, and 2.5 mm in 11-15 years
removal of recommended after 4 to 6 months depending on the fully circumferential complete bone remodelling (woven bone) at the fracture site. The bone healing time is especially dependent on the patient’s age.
